The driver, identified as 44-year-old Rafael Vasquez, was in the self-driving vehicle when it fatally hit 49-year-old Elaine Herzberg as she crossed an unlit Tempe, Ariz., roadway outside of a crosswalk with her bicycle.

Video taken from onboard cameras in a fatal self-driving auto crash in Arizona Sunday night shows the operator did not have her eyes on the road and the pedestrian was visible for at least a second.

A video shot from the vehicle's dashboard camera showed the safety driver looking down, away from the road. She later died in hospital, becoming the first person to be killed by an autonomous vehicle.

"It's actually really boring to drive a vehicle that you're not driving", said Eran Sandhaus, a former vice president at Aptiv PLC who oversaw autonomous driving and is now an adviser to mobility start-ups in Silicon Valley.

"As of March, Uber was struggling to meet its target of 13 miles per "intervention" in Arizona, according to 100 pages of company documents obtained by The New York Times and two people familiar with the company's operations in the Phoenix area but not permitted to speak publicly about it".

Waymo, formerly the self-driving vehicle project of Google, said that in tests on roads in California a year ago, its cars went an average of almost 5,600 miles before the driver had to take control from the computer to steer out of trouble.

The 22-second video released by the Tempe, Arizona, Police Department shows a pedestrian walking from a darkened area onto a street just before an Uber SUV in self-driving mode strikes her. It may be noted here that back in 2017, Arizona made a decision to allow testing of these self-driving cars on the public roads. Most of those vehicles need one, or often more, people in the auto to make sure everything is running smoothly. Hopefully, this isn't an inherent problem with self-driving technology and Uber discovers what went wrong.

"Operating a driverless vehicle on a fixed route could reduce possible confusion for an artificial intelligence-driven auto because it takes time for its computing system to become adapted to a new environment", said 7Starlake Co.

Around the same time, Uber moved from two employees in every auto to one.
